Job Description

Summary:

The Medical Assistant provides support to Gateway*s Children*s Services Division under the umbrella of Gateway*s nursing department and with supervision from the Nurse Supervisor/Trainer. The position*s primary function is to promote the overall psychiatric and physical health of the children served through coordination of care with primary and specialty care providers and communication with families monitoring targeted health indicators and primary care screenings.

Responsibilities:

Gathers information in a medical history regarding a child*s youth*s general health including body mass index sleep issues asthma oral health immunizations tobacco use and exposure communicable diseases and other infectious diseases surgeries etc. accurately documenting it in the electronic medical record.

Completes screenings using standardized tools for substance use and mental health including SBIRT trauma depression anxiety suicidal ideation and social determinants of health contributing information to the person-centered treatment planning process

Monitors completion of required screenings according to policy timelines.

Facilitates the communication of information with community primary and specialty care providers to support integrated care for clients maintaining frequent and timely communication as needed.

Provides relevant education to clients and families about medical and psychiatric conditions under the direction of the healthcare professional.

Communicates with families to reinforce instructions provided by healthcare professionals regarding medication administration dietary restrictions activity levels and other aspects of managing a health condition.

Encourages clients to adhere to prescribed medications and treatments emphasizing the importance of compliance for managing their health condition effectively.

Assists client/family in scheduling appointments and lab work relaying details to the client and assisting with arranging transportation if needed.

Meets with families at the Center and in their homes as needed to perform job responsibilities.

Engages and maintains collaborative positive relationships with a network of specialized community providers to address needs of population including nutrition education physical activities smoking cessation health literacy housing social and rehabilitative programs and job training and placement.

Maintains detailed accurate and timely documentation in medical record.

Communicates information to the care team in a timely manner.

Attends supervision agency and team staff meetings as scheduled.

Enhances and maintains skills through educational activities and trainings.

Engages in trainings on evidence-based practice models specific to their assigned population including motivational interviewing implementing learned skills in daily interventions.

Follows agency policies and standards for client rights and confidentiality.

Assists in monitoring caseloads and required documentation.

Performs other duties as assigned.

Other information:

QUALIFICATION REQUIREMENTS:

Completion of medical assistant training program including internship

Basic Life Support Certification required.

Minimum of 1 year experience preferably working with children and families

Minimum of 1 year working in a behavioral health setting

NECESSARY SPECIAL REQUIREMENTS:

Valid driver*s license and use of car to complete major functions of job.

Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.

CPR training within 30 working days of employment and maintained annually.

JOB KNOWLEDGE SKILL AND ABILITY:

Understanding of mental health and substance use diagnoses.

Understanding of family systems model of care

Knowledge of community resources

Familiarity with screening/ assessment tools and evidence-based practices relevant to position

Oral and written communication skills at a proficient level ensuring effective and professional communication.
