Position Summary:

Under the direction of a Registered Nurse, the Licensed Practical Nurse (LPN) participates in direct and indirect patient care and health teaching to the full extent of his/her generic and continuing education,competenceand experience.The LPN may delegate duties to other caregivers as required/permitted and will consistently provide appropriate supervision.The LPN works with other staff membersin order tomeet the goals, objectives and standards of the unit and contributes to the attainment of divisional and system wide goals and objectives.


Major Responsibilities:

Participates in the assessment of the patient's health status by collecting and recording data from documented information, observation, patient/significant otherinterviewand other health professionals.

Participates in the nursing diagnosis and patient needs based on assessment data.Includes physical, psychosocial, cultural, educational, and spiritual needs as applicable.

Participates in the identification of expected patient outcomes/goals.

Provides input into patient or caregiver learning needs.As directed by the Registered Nurse, documents an educational plan that demonstrates consideration of the patients age, cognitive level, culture, and preferred method oflearning;i.e., printed matter, video's and/or demonstration.

Documents patient response to interventions/nursing care.

Includes patient and/or significant other in planning for care.

Administers all therapeutic interventions in a safe and efficient manner.

Administers medication accurately, observing patient response, as evidenced by documentation in the medical record and the lack of negative outcomes.

Triages phone calls and executes appropriate follow through.

Assists physicians with treatments, procedures, and special tests as required, participating in the setup, maintenance, and disassembly and cleaning of patientequipment

Demonstrates a commitment to promote an office environment that supports positive public relations and image, effective communication with patients, visitors, and health team members.

May delegate selected patient care activities to other licensed practical nursing staff and/or MA staff.Delegation is appropriate to the level of training and competence of staff and the patient need.

Responds promptly,professionallyand calmly to emergency situations.

Interacts and copes effectively with diverse populations in stressfulsituations

Demonstrates effective communications methods and skills, utilizing appropriate reporting channels.

Participates in the orientation of new staff and acts as a preceptor when assigned.Completes orientation paperwork for new staff as outlined in hospital procedures.

Completes all unit task sheets or checklists as required.

Follows hospital and department policies and procedures.

Actively participates in departmental performance improvement and training activities.

Acts as a role model for all clinical staff. Trains, directs, and supervises medical assistants.

Participates in the interviewing and evaluating of the medicalassistants

Meets established productivity standards.

Facilitates and promotes the sharing of knowledge and content throughout departments.

Takes responsibility for ensuring that all work outcomes satisfy the UMass Memorial Health System True North.

The individual must support the mission, vision, and goals of HealthAlliance-Clinton Hospital and serve as a role model for CARES values.

Adheres to change control processes.

Participates in cross training to optimize department resources.

Demonstrates excellent attendance and actively participates in a variety of meetings and training sessions as required.

Position Qualifications:

Ability to perform detailed assignments with minimal supervision and work under pressure.

Knowledge of the nursing process for the delivery and management of patient care from admission to discharge.

Ability to createdocumentationwhich is accurate, legible, and concise.

Ability to complete and maintain all required licensed practicalnursescompetencies.

Ability to read, write and speak clearly in English.

EDUCATION:

Successful completion of an accredited Licensed Practical Nurse Program.

EXPERIENCE:

Prior experience in direct patient care preferred.

LICENSES, CERTIFICATIONS, ETC.:

Current licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts.Current BLS and CPR Certification
