Description

The Health Equity Program Manager is responsible for supporting program development, planning and implementation, communications, monitoring, and evaluation of programs to promote health equity. This position uses data to identify health disparities and engages community, member and Neighborhood resources to develop innovative programming to address disparities. This position applies project management principles to facilitate program implementation. Provides regular reporting and updates to committees and senior leadership to highlight progress and ensure alignment with company initiatives and goals.

Duties and Responsibilities:

Responsibilities include, but are not limited to:

  • Lead the design, implementation and evaluation of health equity initiatives as part of population health and quality efforts. Partner with internal and external stakeholders to assist in the design, implementation, and evaluation of disparities initiatives.
  • Convene internal and external groups to ensure member, provider and community representation in health equity initiatives
  • Lead cross-functional workgroups to guide program design, implementation and evaluation
  • Gather and analyze data to support decision making and program measurement
  • Present health equity program plans and evaluations to senior leadership
  • Engage in continuous quality improvement efforts to support the monitoring, evaluation and improvement of health equity initiatives
  • Ensure alignment with health equity accreditation standards
  • Manage the implementation of health equity initiatives using project management principles. Provide regular reporting on implementation progress to senior leadership
  • Understands complexity within functions, relationships, and contracted obligations with ability to balance completing priorities
  • Takes responsibility for professional development, supports a learning environment and meets professional competency requirements
  • Represent Neighborhood at community forums related to health equity.
  • Other duties as assigned
  • Corporate Compliance Responsibility - As an essential function, responsible for complying with Neighborhood's Corporate Compliance Program, Standards of Business Conduct, applicable contracts, laws, rules and regulations, policies, and procedures as it applies to individual job duties, the department, and the Company. This position must exercise due diligence to prevent, detect, and report unlawful and/or unethical conduct by fellow co-workers, professional affiliates and/or agents.
Qualifications

Qualifications

Required:

  • BA/BS degree or minimum of five (5) years of relevant community experience in organizing or supporting at-risk and vulnerable populations in lieu of a degree
  • Minimum two (2) years experience in managed care or health care setting
  • Deep knowledge of cultural competency and historical traumas within the United States healthcare delivery system and challenges serving Medicaid Members.
  • Strong program/project management, analytical, planning, and goal-setting skills. Including process improvement and change management
  • Ability to develop and implement detailed project plans, including handling multiple projects simultaneously in a deadline driven environment
  • Experience working with multiple collaborators and communicating with all levels of various organizations
  • Knowledge and experience in quality improvement and quality initiatives
  • Strong organizational and documentation skills
  • Solid time management and organizational skills
  • Ability to work autonomously and independently
  • Excellent customer service orientation
  • Intermediate to Advanced skills in Microsoft Office (Project, Word, Excel, PowerPoint, Outlook)

Preferred:

  • Graduate degree in public health, policy, heath care, or other related field
  • Community experience in organizing or supporting at risk and vulnerable populations
  • Five (5)+ years' experience working in a managed care environment with focus on quality improvement, health care analytics and quality improvement requirements
  • Two (2)+ years' experience in the analysis and interpretation of health care data, such as health care claims data, public health surveillance data, health survey data and clinical data abstracted from medical records
